School list help pls (3.99/516/ORM)

Hi folks, making this post on behalf of my friend who needs some advice on their school list.

Stats:

• ORM 3.99 GPA | MCAT 516 (130/130/127/129)
• 800 hours clinical (EMT, had a leadership role as well)
• 3000+ hours as a research assistant (no publications)
• 100 hours shadowing
• 100 hours non-clinical volunteer
• 500 hours TA

Schools (MD only):

• Shooting for T20, Georgetown

Questions:

• Is it ok having such a research-heavy app and only applying to MD?
• Should they be worried about their MCAT for T20 as an ORM?
• Any other school recommendations?
