Sketchy Derm Offer or Not So Bad?
New grad offer MCOL area. Derm (speciality of choice, but getting less motivated towards it with these crazy offers). 1 hour commute so 2 hours driving total everyday. However, only 4 day week schedule.
I know this is a lot. I appreciate anyone who can take the time to read this/offer any advice.
a. Base Salary: $35 per hour for first 6 months during training period. First of the month following the 6-month training period will move to a salary of $100,000 per year
- Bonus potential for the first year following her 6-month training period: 20% of physician assistants net service receipts collected during such calendar year with a threshold between $500,000- $650,000 and 25% of Net Service Receipts collected during such calendar year in excess of $650,000 for General Dermatology and Cosmetics. The bonus payment should be paid monthly.
c. Second year following her 6 month training period: guarantee salary goes away and she is eligible for 25% of PAs net service receipts collected during such calendar year. This amount should be paid in the month following the month in which the NSR are collected.
d. Other Business Points:
- Work Time: 4 days per week. Hours per Day: 9 hours patient visit time. Patient Visits: Expectation minimum 30 patients per day.
- Personal Time Off: 3 weeks per year (12 days) following her start date then 6 weeks of unpaid time off per year commencing the 2nd year following training completion.
- Holiday Time Off: Same as other employees of practice
- CME Time Off: Lumped into unpaid time off
- CME Allowance: Up to $1000 per year towards CME and related travel
- Paid health licensure, society dues and any regulatory fees
9. Paid Malpractice Insurance Upon termination of employment PA shall work with the practice to notify the carrier that any prepaid amounts for coverage are to be reimbursed to the practice. PA shall be responsible for procuring tail coverage upon termination.

I know Derm offers are usually offensive/scammy. This one is not as bad as some I have gotten. I'd appreciate any insight that you may have. As a new grad understanding a commission based salary and how much NSR I'd be generating is pretty challenging. I am also reaching out to other PAs in the clinic to understand how much NSR to expect.
