Why does my ladder cuff do this?

I’ve redone this 6 times and it keeps doing the same thing I’m not sure what I’m doing wrong because I make sure they’re both equal at the end and I’m pulling it as well to make sure it’s sturdy 😭 can anyone help or show me a YouTube video
