Is this a monstera, and what's wrong with it?

Hey houseplantians, novice here. My fiancee and I have had this plant for a couple of years. We have since forgotten the name of the plant, and have had an issue keeping two of the stems healthy enough to hold their leaves. The roots have always been seemingly weak as well, as they are easy to move in the soil. Any insight would be greatly appreciated.

We just moved it to a bit sunnier part of the room, and are planning on re-potting our plants tomorrow.
