Horse cutter and bleeding cuts

Can someone explain how accumulation damage works? I originally thought it hits a hidden meter like darksouls due to how poison build up affects the pc but seems to just do percentage damage? Or is this different with +10% bleed damage in the unique is there a threshold where they start getting DoT?
