Why is the middle one "sunken in"?

So these are from my last relapse about 6mo old, they were pretty deep and I'm guessing it went as far as the dermis. The ones on the side are a liiiittle bit raised, but mostly flat. However the one in the middle is "sunken", as you can see better on the 2nd pic. Why is that?
