Printing on ruled index cards?

I bought some 4x6 index cards to use for storing recipes and was hoping to print them out on the index cards before laminating. Is there any way to format the page so the text is on the ruled lines on the index cards? The first line is 0.5" from the top, and then every line after that is spaced 0.25"
