What does this MRI result mean. Did I re herniate????

Just got a 5 week post op MRI result. Can anyone decipher?

Status post recent L4-L5 posterior decompression and discectomy.

Findings most compatible with a recurrent left subarticular zone disc extrusion (doubt postoperative hematoma). This results in severe left lateral recess narrowing and moderate central canal stenosis. This study has been designated as a OP MD Notification Report. Narrative EXAM: MRI LUMBAR SPINE WITH AND WITHOUT CONTRAST INDICATION: M54.40 Lumbago with sciatica, unspecified side 110; additional clinical history per Epic: "S/p L4/5 METRx on 11/26. He was doing great but then started having severe emesis from norovirus and began to have recurrent left leg numbness "

COMPARISON: Lumbar spine MRI, 11/25/2024. TECHNIQUE: Multiplanar, multisequence MR imaging of the lumbar spine was performed both with and without contrast administration (9 cc of Gadavist). FINDINGS: Note: For the purposes of this report, 5 lumbar type vertebral bodies are presumed. Conus medullaris / nerve roots: Tip of the conus terminates at the level of T12-L1. No abnormal signal in the conus medullaris. No suspicious leptomeningeal or epidural enhancement. Alignment: No malalignment. Vertebrae / Discs / Facets: Status post recent left laminectomy and medial facetectomy for excision of a very large central disc extrusion. Along the left subarticular zone, there is now a rounded focus of signal abnormality that appears contiguous with the disc (at least on sagittal T1 image #7). This is isointense to disc signal at the 1 and hyperintense signal on T2/STIR (suspect nucleus pulposus/edematous recurrent herniation). There is peripheral enhancement around the site of severe left lateral recess narrowing. There is very mild left foraminal narrowing. The degree of congenital canal narrowing is much less than the initial preoperative Mrt. mouon is moderate (AP diameter: ~8mm). Vertebral body heights are maintained. Chronic degenerative fatty endplate changes at L4-5. Few small Schmorl nodes. No significant marrow edema. Level by level degenerative changes as recently described, not significantly changed aside from at L4-L5. Briefly, there are small symmetric disc bulges at T12-L1 through L2-L3 without significant foraminal, lateral recess or central canal stenoses at these levels. Additionally, there is a symmetric disc bulge and central annular fissure with mild facet arthropathy, but no significant foraminal, lateral recess, or central canal narrowing. Additional comments: Expected postoperative changes in the paraspinal soft tissues without a large drainable fluid collection.
