Help me find this Spiderman character please

For years I've had what happened to this female villain pop into my head every now and then because I was really young when I read it and it stuck with me, basically, this villain couple are fighting spider man, one can stop tech from working, the other can make forcefields. I can't remember exactly who fired it but a blaster is fired at her and either she or he puts up a forcefield and the shot gets stuck in there with her, I don't remember if it killed her but it definitely disfigured her horribly, anyone know who she is and what issue this was, and if she survived in it?
