Mutual releases in S.A. with carrier

Hi All. I keep getting push back on a settlement agreement with the insurance carrier's counsel on mutual releases. Defendant didn't file counterclaims, so it says "no releases by defendant." Isn't this common practice? Why would they not just agree to mutual releases? Any insight would be appreciated. Thank you.
