Is this fin rot?

I noticed a while ago that my goldfish’s dorsal fin disappeared. I figured it would just grow back, but he began developing black spots. I’ve been doing constant water changes, but it seems to be getting worse. He has also lost his appetite. From the internet, I believe this is a very late stage of fin rot. Is this fin rot? Can he be saved? What treatments do you recommend?
